IMPORTANT INFORMATION ABOUT OPENING A NEW ACCOUNT.

Federal law requires all financial institutions to obtain, verify and record information that identifies each person who opens an account. When Client opens an account, Bank will ask for Client's name, address, date of birth, or other appropriate information that will allow Bank to identify Client. Bank may also ask to see Client’s driver’s license or other identifying documents.

I, the undersigned Client or Clients (who are referred to as the "Client" for convenience), hereby establish a custodial account (the "Account") under the Terms and Conditions of the Evolve Bank & Trust Custody Agreement. I designate Evolve Bank & Trust ("Bank") as Custodian of the Account and make the following declarations.

U.S. Citizen Certification: I, the Client, hereby certify that each Client is a U.S. citizen or entity.
Resident Alien: I, the Client, am a U.S. Resident Alien as defined by the Internal Revenue Service (IRS). I agree to notify you immediately if my resident status changes. (An Account cannot be established for a non-resident alien.)
